Choose the Record Custodian

Kentucky does not have one office that holds every release record. The correct contact depends on the last custodian and the record needed. A county jail release log stays with its local custodian, while a state-prison movement or discharge record belongs to KDOC. Parole decisions, supervision details, trial-court files, and federal custody records follow separate routes.

KDOC requires a written request for offender records. Include a mailing address and identify the person with a full name plus date of birth, PID, or DOC number. The standard form is optional.

KDOC open-records instructions

A written Kentucky Parole Board decision must be requested through the Board's open-records route. Questions about an active supervision record go to the district shown in KOOL and the KDOC district contact list. Trial-court records are different: the Kentucky Court of Justice directs Circuit, District, Family, and Business Court requests to the circuit clerk in the county that handled the case.
